The practice has put in place a policy that requires all patients turning 16 to have access to their own medical records. It may be that until now your parent or carer had access to them but this will stop once you have reached your 16th birthday.
To gain access to your online records please complete the appropriate Registration Form and come and see the secretary at the surgery bringing with you a photographic ID of yourself.
If the child is felt not to be ‘Gillick competent’ then the parent or carer will still have access to their online medical records.
